Responsibilities:
- Conduct comprehensive assessments and evaluations during homecare visits to ensure high-quality care delivery.
- Manage admissions, discharges, recertifications, and reassessments for clients in adherence to established policies and procedures.
- Collaborate with interdisciplinary teams to develop personalized care plans based on clients' unique needs, goals, and preferences.
- Oversee and coordinate the delivery of skilled nursing services, therapies, and necessary interventions for excellent client outcomes.
- Provide education and support to clients and families, empowering them in disease management, medication administration, and self-care techniques.
- Act as a liaison between clients, families, healthcare providers, and community resources for seamless continuity of care.
- Train and supervise homecare staff, ensuring compliance with standards of care, policies, and regulations.
Qualifications:
- Valid Registered Nurse (RN) license in the state of Minnesota.
- 2 years of experience in homecare or community health nursing.
- Demonstrated leadership and management skills, with the ability to inspire and mentor staff effectively.
- Excellent clinical assessment and critical thinking abilities, coupled with a passion for providing exceptional patient care.
- Strong communication and interpersonal skills to establish rapport with clients, families, and healthcare professionals.
