What are the responsibilities and job description for the Case Manager position at LESC?
BASIC FUNCTION: Provide case management services with a focus on recovery and permanent housing to 20 tenants.
MAJOR DUTIES:
· Case management services will include developing and addressing service plan goals that will be based on integrating the participant into the community for continual support and to successfully identify permanent housing.
· The Case Manager will develop an individualized housing-related needs assessment and support service plan, including an action plan with clearly stated goals and outcomes that address client's access to preventive, ongoing, and mental health treatment services .
· The Case Manager will directly provide: case management, medication management, rehabilitation, personal assistance that emphasizes learning daily living skills, residential stability in housing, financial management and assistance in gaining access to appropriate public benefits and services, peer support and be on call 24 hour/7days per week
· Case Manager will focus on each client's recovery to his or her fullest potential by providing referrals to educational opportunities, job readiness skills, vocational training and employment.
· Case Manager will provide individualized budget assistance that promotes tenant's learning to pay rent in a timely manner in order to maximize chances for successful transition into independent living.
· Conduct once monthly home visit and annual apartment inspections.
SCHEDULE: Full Time
RATE: $49,500 / year - $50,430 / year dependent on experience
LOCATION: Bronx, NY 10460

QUALIFICATIONS:
- Case manager must either have a bachelor degree or CASAC with relevant experience.
- Must have excellent assessment and crisis intervention skills, and knowledge of resources/entitlements. Experience with individual treatment and group work required. Must be organized, self-motivated, a team player, and computer literate.
- Familiarity with Microsoft Office and Foothold
- Ability to write clear and concise progress notes
- Ability to establish rapport and engage clients in participating in their own treatment
