The Family Reunification Case Manager coordinates individual cases and personal development plans to provide tailored housing supports for families transitioning out of child welfare system into safe, affordable, long-term housing within the Department of Children, Youth and Families (DCYF) Family Reunification Homecoming Program. The Case Manager will help connect the existing child welfare clients to the housing resources/services to ensure parents/clients are informed about housing services and the housing search; supporting clients with their housing search, or providing housing options. Once housing is obtained the Case Manager will work with individuals as they create a personal development plan to measure progress and growth of social skills and life skills, establishing and implanting basic household routines, cooking on a budget, parenting education, etc. This position is required to maintain accurate records and provide detailed client progress reports to the Program Coordinator.
Qualifications:
High school diploma, or GED minimum
Prefer AA Degree with emphasis in social services or closely related field.
Knowledge and experience working with children/families in the child welfare system, and disadvantage populations.
Verbal and written fluency in language(s) required by program.
Act as reporting agent for suspected child/adult abuse and neglect
