Responsibilities
The Travel Home Health Nurse (RN) provides high-quality in-home health care as a full-time employee to our patients in agencies across 38 states, from California to New York. Responsibilities include but are not limited to :
• Works to reduce unnecessary patient hospitalizations, improve patient safety, and implements processes and best practices to ensure positive patient outcomes.
• Makes the initial and/or comprehensive nursing evaluation visit, determines primary focus of care, develops the plan of care within State specific guidelines, and submits accurate, complete, and timely documentation, per policy.
• Initiates, develops, implements and makes necessary revisions to the plan of care in collaboration with the physician and other health care professional's involved in care.
Education and Experience
License Requirements
• Must have 1 year of recent Home Health Registered Nurse experience in the last 12 months
• Must have 1 year of recent Homecare Homebase (Point Care) experience in the last 12 months.
• Current American Heart Association BLS
