JOB DESCRIPTION
- Position : CalAIM Community Care Coordinator
- Pay Range : $22-$25

POSITION SUMMARY
The CalAIM Community Coordinator is responsible for processing the ALWP beneficiary
application as per Regulatory Agency and Libertana Home Health Policy. This includes
with the beneficiary.
QUALIFICATIONS
1. Exercises excellent judgment and has good organizational skills.
2. Fluent in English. Possesses effective written and verbal communication skills.
3. Proficient in the use of computers, electronic medical records, and Microsoft
Office.
4. Ability to establish and maintain good communication and relationships with
office, field, and administrative personnel.
5. Is self-directed with the ability to work with little supervision.
6. Must be a licensed driver with an automobile that is insured and is in good
working order, in accordance with state and / or organizational requirements.
7. . Knowledge of the State of California Assisted Living Waiver Program Care
Coordination Agency Provider Handbook, RCFE Provider Handbook and Title 22
Division 6 for RCFE.
8. Bachelor’s Degree in Health Care related field preferred.
9. Knowledge of community and housing resources and government benefits / welfare
system.
10. Possesses current CPR certification.
11. Current and satisfactory report on pre-employment physical examination including TB
Screening Test or chest X-ray as required by Agency policies and procedures. Must be
free from signs of infection and illness.
ESSENTIAL DUTIES AND RESPONSIBILITIES
1. Engage eligible members in the community by performing home outreach
2. Assists in assigning caseloads / clients for ECM Case Managers
3. Maintains proper charting, progress notes and case records for each enrolled Member,
including chart creation and upkeep, and other charting tasks as assigned
4. Responsible for securing authorization and checking Medi-Cal for eligibility
5. Assists clients in enrolling in and recertifying Medi-Cal when necessary
6. Assists Medi-Cal beneficiaries in obtaining access to waiver services, state plan
services and other community resources by identifying, coordinating and collecting
data required for the ALWP application.
7. Responsible for the proper completion of all documents required for the State
Waiver Program Application as per ALWP and Agency policy.
8. Reviews and evaluates the ALWP documents necessary for State Waiver Program
Application, including Initial Assessments, Re-assessments, ISPs, Physician’s
Reports Medi-Cal eligibility, etc.
9. Coordinate with ECM Lead Case Manager when care coordination is necessary
10. Complete a bio-psycho-social assessment and a Care Plan. Oversees the implementation
of the Care Plan.
11. Communicate with mental health, substance abuse and medical provider to ensure
continuity of care.
12. Connect Member to other social services and supports the Member may need, including
transportation.
13. Advocate on behalf of Members with health care professionals.
14. Use motivational interviewing, trauma- informed care, and harm-reduction approaches.
15. Develop effective, trusting relationships with clients with focus on facilitating their
independence and long-term housing stability.
16. Facilitate linkage to mental health, substance abuse and medical services and provide
transportation when needed.
17. Accompany member to office visits, as needed and according to MCP guidelines.
18. Monitors service delivery, adhering to the prescribed schedule of client contact. Conducts
face to face visit with Members, as needed.
19. Handles complaints from clients, families or friends.
20. Reports all signs of abuse or neglect
21. Participates in Education, Training and Quality Improvement Activities. Assists the
Agency in maintaining compliance with Federal, State, Local and HIPAA Regulations or
Joint Commission Standards.
22. Establishes and maintains good relationships with all Health Plans and other vendors.
23. Performs other duties that may be assigned.
