What are the responsibilities and job description for the CDPAP Coordinator position at Liberty Home Care?
Liberty Home Care is looking to add a CDPAP Coordinator to our dedicated home care team. The CDPAPs coordinator is responsible for the program maintenance and account management of the Consumer Directed Personal Assistance Program (CDPAP).
Duties & Responsibilities
- Collaborate with outreach team to increase continued growth of the program
- Manage incoming referrals, authorizations etc. and enter into EMR (Electronic Medical Record)
- Monitor CellTrak portal and clear alerts and exceptions as needed
- Process payroll
- Receive calls regarding payroll, CellTrak, issues or scheduling problems.
- Provide follow up with care manager at insurance company with any scheduling issues.
- Discharge consumers and assistants including processing data change forms.
- Ensure all information/authorizations/documentation required for billing claims to insurance provider(s) is collected, completed and updated.
- Assist with walk-ins or CellTrak training and set up.
- On Call rotation may be required with the LHCSA scheduling team
Required Qualifications
- High school diploma or equivalent required.
- Valid NY state Driver’s license and use of personal vehicle for local business travel
- Ability to work on-call responsibilities as assigned as well as occasional evening, weekend and holidays
