What is Lifebanc?

Lifebanc, is the federally designated organ, eye and tissue recovery organization that serves 20 counties in Northeast Ohio. Put simply, we facilitate and coordinate organ, eye and tissue donation and transplantation in our community and educate the public on the importance of our mission. Position Description:

The Family Support Liaison is responsible for ensuring that all donors and potential donor families are offered the opportunity for donation through the collaboration with Lifebanc and hospital staff members. The Family Support Liaison (FSL) serves as a resource and support mechanism for both the donor families and hospital staff during the active donation process and responds to all donor family needs 24/7, in addition, the FSL will support and maintain the mission, vision and values of Lifebanc in their work and job duties.


Essential Functions:

  • Serves as a member of the health care team and provides support for end-of-life decisions regarding organ, tissue, and eye donation.
    • Provides availability beyond that of a typical work week to include weekend and holiday call shifts in 12-hour increments.
    • Onsite presence for collaboration with the health care team during end-of-life discussions of potential organ, tissue, and eye donors.
    • Identifies and contacts appropriate next of kin of potential organ, tissue, and eye donors in compliance with regulatory standards and Lifebanc policies.
    • Discusses the opportunity of organ, tissue, and eye donation with next of kin and obtains authorization/disclosure for organ, tissue, and eye donation.
  • Obtains and documents a comprehensive medical and social history interview with the next of kin, significant others and/or healthcare professionals of potential organ and tissue donors in compliance with CMS, AATB, FDA, EBAA, AOPO standards and Lifebanc policies.
    • Accurately completes the appropriate electronic Donor Risk Assessment Interview in iTransplant.
    • Accurately documents required information in iTransplant in a timely manner.
  • Provides crisis intervention, counseling techniques and emotional support for the donor family during the organ donation and recovery process in compliance with Lifebanc policies.
    • Provides support and memory making for donor families during the donor case.
    • Supports next of kin with requested follow up such as acceptance/deferral outcomes and/or post recovery notification.
  • Ensures departmental quality guidelines are maintained through timely and accurate documentation in donor charts to maximize opportunities for transplantation and provides essential customer service both within Lifebanc and outside the organization. 
    • Completes accurate documentation in iTransplant.
    • Accurate, timely completion of charting error tasks assigned in iTransplant and responds to all other inquiries from the Quality Department required for donor chart clearance.
    • Delivers customer service with the highest level of excellence to all individuals within and outside of Lifebanc during routine and stressful situations.
  • Maintains an individual authorization rate that meets the department’s benchmarks.
    • Attends and actively participates in mandatory quarterly consent trainings.
  • Assists the Bereavement Department and other departments in donor family related needs.
    • Provides accurate reporting and updates to the Bereavement Department regarding after care services to donor families.

                            

Qualifications:

  • Bachelor’s Degree preferred in social work, psychology, health care, funeral industry, education or ministry.
  • At least 4 years of progressive experience in related field, preferable in healthcare.
  • Experience in dealing with families in grief and/or crisis situations.
  • Ability to work independently, to exercise initiative and discretion, and exercise sound judgment.
  • Ability to communicate effectively with a wide range of people including families, physicians, nurses, clergy and staff regarding sensitive issues.
  • Ability to make decisions after analyzing and interpreting a situation.
  • Must travel throughout NE Ohio, be available during day/night hours including weekends, holidays and have the flexibility to work a 12 hour on-call rotation schedule.
