JOB SUMMARY
Ensures that all services are delivered in accordance with the Mission Statement and S.P.I.R.I.T. Values of Carroll Hospice and Carroll Hospital Center. Provides professional, comprehensive, family-oriented nursing care to individuals in the home setting during the final stages of life. Assesses patients' and families' needs and formulates, implements and evaluates plan of care. Performs primary care case management function for assigned patients.

SPECIFIC REQUIREMENTS
Fleet Safety Requirements. Interpersonal communications skills (written and oral) in order to educate and comfort patients and their families. Maintain effective contacts with a variety of hospital personnel. Maintain appropriate documentation records. CHPN certification should be within 4 years of hire or after meeting CHPN exam requirements of 500 hours of hospice and palliative nursing practice in the most recent 12 months or 1000 hours of hospice and palliative nursing practice in the most recent 24 months (required to apply for CHPN exam). Professional licensure required in Maryland and Pennsylvania

REQUIREMENTS
- Required Associate's Degree Successful completion and graduation from an accredited school of nursing.
- Preferred Bachelor's Degree
- 2-4 years Requires minimum of two years of recent medical/surgical nursing experience. One year home health experience preferred. Expertise in performing health assessments. Required
