Position Title: LPN
Reports to: RN Supervisor
Position Summary:
This position is responsible for developing and providing nursing services as prescribed by the nursing care plan and within the framework of the administrative structure of the agency, nursing standards of practice and the program policies of practice. The LPN will also be responsible for providing supervision to the Home Health Aide staff within the Home Care Program. The LPN will evaluate the client using the holistic model, which includes the physical, psychosocial, spiritual, and environmental realms, within the context of the Community Health Model. This position works with clients individually to assess client needs and services upon completion of the assessment submits documentation of need. The LPN will assist other department staff with activities such as client follow-up, field staff in-services and covering the scheduling of services as needed. Work performed in this position will be of a professional and confidential nature. Work performed in this position will be of a professional and confidential nature. Work requires an attention to detail, flexibility and the ability to handle multiple functions at one time. This position will have frequent contact with agency personnel and clients of the agency. A positive and cooperative image and attitude is crucial for this position. Work performed is under the general supervision of the RN Supervisor – Help at Home and in conjunction with other agency staff. This position requires a significant amount of independence, self-direction, and time management to accomplish its essential functions and responsibilities.
Principle Responsibilities/essential functions:

  • Responsible for providing nursing services. This includes conducting admission assessments to determine the appropriate level of care, making home visits to home care clients, developing service plan, providing resources, making referrals, completing all required admission documents, providing direct care to private duty clients such as injections, venipuncture, and teaching clients and caregivers.
  • Responsible for providing supervision. This includes direct supervision to the home health aide staff and indirect supervision to the homemaker staff. Conducting evaluation, ongoing training, problem solving, and review of documentation for the home health aide staff.
  • Responsible for completing required documents for a client admission and/or discharge. This includes the completion of time sheets, mileage sheets, nursing service reports, patient and nursing assessment forms, activities of daily living and instrumental activities of daily living forms, medication sheets, in home charts, consent forms, financial responsibility forms, funding documents, home environment forms, home health aide and homemaker care plans in a timely manner.
  • Responsible for interviewing and hiring home health aide staff. This includes completing interviews with applicants and checking references as needed.
  • Responsible for orienting new home health aide staff. This includes covering department policies and procedures, service records, CPR training, map reading, infection control and field training.
  • Responsible for performing other department or agency functions as necessary. This includes attending meetings, adult immunizations, chart audits, assist Assistant Director with reports and audits, answering department phones and performing personal care due to staff shortages.

Working Conditions:
Work is primarily in agency office and in client homes. Work requires employee to drive in all types of weather conditions. Work is generally between 8:00am and 5:00pm and evening and weekend assignments that involve overtime may occasionally be required. Part-Time position, not exceeding 15 hours per week.
Qualifications:
A current LPN license for the state of Ohio. Minimum of three years supervisory experience. Must possess a valid Ohio driver’s license, have a vehicle available for agency use and maintain adequate insurance. Maintain personal professional liability insurance in the amount of one million dollars. Employee must have the ability to lift 30 pounds.
Knowledge, Skills, and Abilities:
General knowledge of and ability to apply public health nursing principles and practices
General knowledge of pharmacology
Possess a wide range of nursing skills and motor dexterity in relation to medical-surgical nursing procedures
Ability accept responsibility and understand limitations professionally, legally, and ethically
Ability to work with minimum supervision
Ability to exercise professional judgement and make meaningful observations and evaluations
Ability to develop and maintain effective working relationships with LifeCare staff, clients, and client caregivers, other community health and social services, and physicians
Ability to adequately document pertinent information on appropriate client forms
Completes and returns all appropriate documentation within the time period established by program policies and procedures.

Job Type: Part-time

Pay: $27.00 per hour
