Job Description: Licensed Practical & Vocational Nurse (Hospice) - Lifelong Hospice Care Inc

Lifelong Hospice Care Inc is seeking compassionate and experienced Licensed Practical & Vocational Nurses to join our dedicated team. As a vital member of our hospice care unit, you will provide high-quality, personalized care to patients and their families during their end-of-life journey.

Basic Requirements:

  • Current LPN/LVN license in the state of Victorville, CA
  • Minimum of 1 year of hospice care experience
  • CPR certification
  • Strong interpersonal and communication skills
  • Ability to work collaboratively in a multidisciplinary team
  • Empathy and understanding towards patients and families in sensitive situations

Responsibilities:

  • Conduct comprehensive patient assessments
  • Administer medications and treatments as prescribed
  • Collaborate with the interdisciplinary team to develop and implement care plans
  • Provide emotional support and education to patients and families
  • Ensure the highest standards of comfort and dignity for patients
  • Maintain accurate and timely documentation of patient care
