Overview

LifeLong Medical Care is looking for a Recovery Support Counselor (RSC) for our Adult Reentry Program (ARP) in Oakland. This position provides substance use disorder/co-occurring disorder (SUD/COD) assessments and counseling services, transition planning, comprehensive case management, and recovery support services to sentenced adults in the criminal justice system with an SUD and/or co-occurring substance use and mental health disorder, who are returning to their families and community following a period of incarceration

 

This is a grant funded, full time, 40 hours/week, benefit eligible position. 

 

This position is represented by SEIU-UHW. Salaries and benefits are set by a collective bargaining agreement (CBA), and an employee in this position must remain a member in good standing of SEIU-UHW, as defined in the CBA.

Benefits

COVID-19 Vaccine Policy

Responsibilities

  • Provides screening and assessments for SUD/COD to individuals within the Santa Rita Jail and at other locations in the community
  • Provides SUD/COD counseling to individuals and groups
  • Provides on-going support and case management services to clients including transition planning, referral to primary care (including HIV, viral Hepatitis (A, B and C) and other infectious diseases), mental health counseling, benefits counseling, food assistance, housing, and referrals for additional SUD/COD treatment, such as Medication Assisted Treatment and residential programs
  • Develops comprehensive service plans and helps coordinate services for/with client and outside referrals services including accessing public benefits and health insurance
  • Communicates with integrated care team members about the program/clients and problems or concerns, including probation officers
  • Complete timely and accurate documentation within EPIC (LifeLong’s electronic health record) including treatment/service/care plans, progress notes, attendance logs, and discharge planning
  • Participate in all required case conferences, clinical supervision, team meetings, outside meetings and trainings as required - in person or virtual

Qualifications

  • Must be certified or registered as an SUD Counselor
  • At least two years’ experience providing SUD counseling and assessments
  • Must pass clearance for Santa Rita Jail
  • Experience working with individuals with involvement in the criminal justice system
  • Lived experience is highly desirable
  • Ability to coordinate, develop, and maintain strong relationships with staff, other providers (including those within Santa Rita jail and Alameda County Probation), and community organizations.
  • Effectively work in a diverse work environment serving a multi-cultural community, work collaboratively with attorneys, case managers, and probation and jail staff
  • Demonstrates ability to work well with people of diverse backgrounds including individuals with mental illness and substance use related issues, those experiencing homelessness, and individuals with a criminal justice background.
  • Demonstrated ability and sensitivity in working with a variety of people from low-income populations, with diverse educational, lifestyle, ethnic and cultural origins
  • Maintain professional code of conduct and confidentiality
  • Maintain a problem solving, solution-oriented attitude in all aspects of work
  • Excellent verbal, written and interpersonal communication skills. Strong organizational, administrative and problem-solving skills, and ability to be flexible and adaptive to change
  • Proficient computer skills including using email, Microsoft Word, and the internet. Experience with an Electronic Health Record is helpful.
  • Must have reliable transportation to travel between sites in a timely manner, e.g. to Santa Rita Jail.
