What are the responsibilities and job description for the Case Manager - Open Access position at LifePath Systems?

GENERAL DESCRIPTION

Triages individuals seeking outpatient services meet timely and effectively and navigate appropriate service options available. Provides initial screenings, crisis intervention and determines clinical and financial eligibility via telephone, zoom, or face to face for Open Access clinics. Completes detailed documentation in a timely manner on all services provided. Develops person driven recovery plans and recommends appropriate level of care. Links individuals to appropriate natural and community resources to meet ongoing needs, as necessary.  Ensures compliant with TAC requirements/rules with all services provided. Knowledge and adherence to trauma informed perspective is required. This person must be efficient in EHR, have ability to multitask, and must be able to provide exceptional customer service in a busy setting. This position will provide support to the Access Center as needed.This role is a key team player position in a constantly changing busy environment that requires accuracy, attention to details, initiative and a pro-active approach to duties and responsibilities while working with the individuals in services, clinical team, and co-workers. The qualified applicant must demonstrate a positive attitude, considerable latitude for independent judgment, as well as professional ethics, appearance, and conduct.

EDUCATION, TRAINING, & EXPERIENCE

  • Bachelor’s Degree in psychology, sociology, or related field from an accredited college or university as required to qualify as a Qualified Mental Health Professional (QMHP).
  • Must have at least 2 years’ experience providing mental health services.
  • Experience in providing tele-mental health services/coaching services highly desired.
  • Experience in crisis response strongly preferred

ESSENTIAL TASKS & STANDARDS

  • Provide brief mental health screening, C-SSRS, ANSA/CANS, TRR, and crisis services to assess needs to determine clinical eligibility for mental health services.
  • Provide quality customer service.
  • Complete documentation in a timely manner on all services provided.
  • Understands and implements trauma informed care practices.
  • Demonstrates effective problem-solving skills.
  • Provide Crisis intervention services to individuals as medically necessary by tele health or face-to-face interventions.

KNOWLEDGE, SKILLS, & ABILITIES

  • Excellent knowledge in tele-platforms, including exceptional ability to engage via multiple tele platforms and guide individuals in tele set-up.
  • Knowledge of the social services system and the ability to connect individual with appropriate community and natural resources.
  • Knowledge of strengths-based person-centered approach.
  • Ability to write clearly and professionally on all services and internal communication.
  • Ability to think clearly and logically to navigate potentially dangerous and/or emotional situations.
  • Ability to respond to individuals with patience, empathy and understanding.
  • Organized, detail oriented, and computer proficient.
  • Demonstrates strong multi-cultural competency.
  • Bilingual (English/Spanish) preferred

CERTIFICATES, LICENSES, & REGISTRATIONS

  • Must have valid Texas Driver License and a good driving record.
  • Must be able to pass a criminal background check.
  • Must be able to complete all LifePath Systems training.
  • Must be able to provide at least 3 professional or educational references. 

BENEFITS

Salary Range: $40,000 per year             

Shift Hours: Monday - Friday 8am-5pm
