Grow and develop with LifeScape! LifeScape has a unique Social Worker Unscheduled Part Time (UPT) opening for our Specialty Hospital and is seeking a passionate and organized individual. This position provides individualized services to children and their families, including family support, service coordination, advocacy, admission planning, discharge planning, and information and referral. Some of the responsibilities for this role include, but are not limited to:
- Maintains and distributes meal tickets for families in need in the Specialty Hospital
- Assists with volunteer coordination and activities for the Specialty Hospital
- Assists in scheduling the on-site apartment, if available, for family stays, and assists families in identifying other areas to stay if needed
- Write discharge assessment in the electronic record for all rehab patients
- Works closely with rehab team, and rehab physicians
- Collaborates with Care Coordinator/Nursing staff
- Documents all contacts with family/guardian, and outside entities
- Assesses and responds to child and family needs for additional services
- Promotes family participation in meetings and activities and assists with family/guardian visits
- Serves as advocate for person supported and their families

EDUCATION AND EXPERIENCE
Bachelor’s Degree in Social Work required. Current license in the State of South Dakota required. Prior experience working with individuals with disabilities preferred.
