LifeSpring has an opening for a full-time Outreach Care Manager position for the PATH Program (Projects for Assistance in Transition from Homelessness). This position is community-based. The Outreach Care Manager will be responsible for developing and maintaining relationships with referral sources including homeless shelters, advocates, food pantries, trustee offices, law enforcement and local business leaders to permit early identification of people who have a serious mental illness/co-occurring disorder and are homeless or at imminent risk of being homeless. They will work with other service providers to identify those who are chronically homeless and will spend extra time attempting to engage them in treatment. The outreach worker will assess the needs of the individual or family and provide case management and necessary referrals for services such as mental health treatment, substance abuse treatment, health care, employment services, securing benefits, etc.. Outreach will be conducted primarily in the local homeless shelters, soup kitchens and on the street. The outreach worker will also serve as a bridge to a permanent care manager. Clients will be linked with housing providers in the area, based on their needs and the challenges present prohibiting successful housing.
The successful applicant will have a Bachelor’s degree in a related field. Reliable transportation is required. Mileage will be reimbursed pursuant to LifeSpring policy. This staff person will work out of the Floyd County office and cover Clark, Floyd, Harrison, Jefferson, Scott, Washington, Orange, and Crawford counties in Indiana.
