Lifestyle Support Services Inc. is

. The position will work with a team of Wellness Coordinators utilizing community based licensed medical personnel to coordinate and monitor all for all individuals residing in residential group homes. This position ultimately works to ensure quality medical care is provided to maximize the health and wellness of all individuals diagnosed with Intellectual Disabilities (ID). Other duties include but not limited to ensuring regulations are met to maintain continued licensure while complying with all corporate policies and procedures, ability to work & interface with physicians of all disciplines, scheduling and attending appointments as required. May require some non-traditional work hours. Annual salary between $45,000-$50,000 based on experience (CNA preferred). Candidates are to obtain ACT 33 and 34 clearances.
