General Summary

Under the direction of the Supervisor, Case Management Services Children's Services, the Wraparound Services (WRAP) Facilitator provides culturally competent and trauma informed services to all consumers, assists consumers to design and implement strategies for obtaining services and supports that are goal-oriented and individualized. The WRAP facilitator shall be responsible for administering the Wraparound planning process as trained by the Michigan Department of Health and Human Services (MDHHS) model. The WRAP Facilitator engages families in the Wraparound process, helping them to formulate a strength- based Family Plan detailing their natural supports, needed resources and goals, plans to accomplish them; and assist them in assembling a Child and Family Team to support this plan. The WRAP Facilitator incorporates LifeWays' mission, vision, and values into all decision-making processes.

Essential Functions

  • Assures that the person-centered planning process takes place and that it results in the individual plan of service (IPOS) as appropriate based on the medical necessity of the individual.
  • Assures that the plan of service identifies what services and supports will be provided, who will provide them, and how the case manager will monitor (i.e., interval of face-to-face contacts) the services and supports identified under each goal and objective.
  • Oversees implementation of the individual plan of service, including supporting the consumer's dreams, goals, and desires for optimizing independence; promoting recovery; and assisting in the development and maintenance of natural supports.
  • Assures the participation of the consumer on an ongoing basis in discussions of his plans, goals, and status.
  • Assures coordination with the consumer's primary and other health care providers to assure continuity of care.
  • Engages with children and families, assisting them to improve their functioning.
  • Provides the Wraparound process with families at a level that is in compliance with the model and meets contractual expectations including unit thresholds and documentation requirements.
  • Interact with clients in non-traditional settings and during non-traditional hours.
  • Represents clients served and through interacting with referral sources.
  • Serves as the gatekeeper for families assigned and provides case management (CSM) and support.
  • Documents services through the use of an Electronic Medical Record and other formats.
  • Represents clients served through interacting with referral sources and other professionals, supporting the child and family.
  • Provides crisis intervention, safety planning and support.
  • Coordinates and assists the consumer in crisis intervention and discharge planning, including community supports after hospitalization.
  • Facilitates the transition (e.g., from inpatient to community services, school to work, dependent to independent living) process, including arrangements for follow-up services.
  • Assists consumers with crisis planning.
  • Identifies the process for after-hours contact. Facilitates services to Individuals and families (up to 12 Individuals) including facilitation and participation in team meetings.
  • Staff may be assigned other CSM cases as appropriate that do not fall under the WRAP designation.
  • Maintain weekly contact with individuals, families and teams.
  • Maintains professional relationships with a variety of Individuals including comfort level working in a wide range of settings (family homes, community settings, schools).
  • Facilitates and participate in monthly team meetings with Individuals and families.
  • Maintains and process required documentation accurately and within required timeframes.
  • Participates in trainings with IPMG Supervisor and/or local state site coach.
  • Participate in WRAP and state required training within 90 days of hire.
  • Completes other projects as assigned by management.
  • Ability to provide services with model adherence.
  • Ability to meet expected case documentation requirements.
  • Ability to maintain compliance with agency and contractual training and licensure requirements.

Required Education/Licensure/Certificate

WRAP I: Possess a bachelor's degree in any field and be a CMHP or be supervised by a CMHP.

WRAP II: Bachelor's degree required in social work, or human services field, and is Fully Licensed Bachelor Social Worker (LBSW). Meets qualifications as a QIDP, QMHP, or CMHP and/or has specialized training in Evidenced Based Practice and experience in the examination, evaluation, and treatment of minors and their families.

Preferred Education/Licensure/Certificate

  • Master's degree in Social Work with either a LLMSW or LMSW licensure with the State of Michigan.
  • Master's degree in Community Counseling with either a LLPC or LPC, licensure with the State of Michigan.
