To help us achieve our mission, we are looking to hire a Psychiatric Mental Health Nurse Practitioner who is interested in increasing the quality of life of our patients and who envisions a world where every aging patient has access to the world’s best healthcare. This is a remote role that requires a California license.
As a key member of our Behavioral Health team, here’s what you’ll do:
Key Outcomes:
- Focus primarily on the Lightyear Health population of seniors with identified or suspected behavioral/mental health conditions and “adopt” these patients to add psychiatric expertise to the medical management plan and mitigate any negative impact that behavioral conditions can have on patients overall health and well-being.
- Treat approximately 12 patients/day independently with minimal MD supervision/collaboration.
- Treat patients via telemedicine who reside at home, in Assisted Living Facilities, Skilled Nursing Facilities, and Independent Living Facilities
- Establish and maintain effective therapeutic relationships with members and their legal representatives, facility staff, PCPs, and specialists to maximize continuity and compliance with individualized member care plans.
- Perform an initial comprehensive assessment on all newly enrolled members and at least annually thereafter. Develops a member management care plan upon enrollment and updates it as needed when changes in condition warrant or following hospitalization.
The most important skills for the role are:
- Current, unrestricted CA NP license
- PMHNP certification from an accredited program
- Minimum of one year clinical PMHNP experience, with home-based care desired
- Fully vaccinated as outlined by our policy

- Knowledge of CPT, ICD-10, and HCPC codes
- Awareness about UM standards, NCQA requirements, CMS guidelines, Milliman guidelines, and Medicaid/Medicare contracts and benefit systems

The clinical team at Lightyear Health is responsible for meeting and complying with our credentialing verifications processes, clinical standards, health screenings, and vaccination policies at all times. It is the responsibility of the practitioner to ensure that your licenses, certifications, and credentials are kept up to date and in good standing.
Anticipated Salary Range
We offer a competitive salary starting at $170k, plus benefits medical, dental, and vision insurance, paid time off, 401k with company matching, and stock options. The exact salary will depend on (1) how many years of relevant and transferable experience the candidate brings to the role and (2) the internal level at which the candidate is hired in relation to our career development framework.
