What are the responsibilities and job description for the Social Worker I - Care Coordination position at Lima Memorial Health System?
Functioning within the Health System’s mission, values, objectives, policies and procedures, the Medical Social Worker intervenes with patients who have complex psychosocial needs, require assistance with eligibility determination for social programs and funding sources, and qualify for community assistance from a variety of special funds and agencies. In addition, the Medical Social Worker offers crisis intervention to patients and families with psychosocial needs and coordinates and facilitates the development of a discharge plan of care for high-risk patient populations. The Medical Social Worker receives referrals for individuals from at-risk populations from interdisciplinary team members (including physicians, case managers, staff nurses, and other members of the care team).
Education: Graduate of an accredited school of Social Work with a Bachelor’s degree is required.
Licensure/Certification: Must be licensed or eligible to be licensed as a Social Worker in the State of Ohio.
Experience: Experience in an acute care setting is preferred.
Skills: Must possess strong interpersonal and communication skills, be motivated and self-directed, and have the ability to integrate numerous job responsibilities.
