LiveWellChoice of Michigan is a new MedicareAdvantage Institutional Special Needs Health Plan specifically available forlong-term nursing home residents. Essential to the success of LiveWell Choiceof Michigan is the patient-centeredNurse Practitioner clinical model.
PositionSummaryNursePractitioners are responsible for effectively medically managing a panel ofLiveWell Choice enrolled nursing home residents in partnership with a primarycare team and deliver the LiveWell Choice Model of Care. Nurse Practitionersconduct a comprehensive assessment, develop and deliver the member’sindividualized plans of care, attend nursing facility care team meetings,communicate frequently with the resident’s family/loved ones and ensure thecare team understands the member’s goals of care and needs. NursePractitioners focus on treating in place, use clinical practice guidelines toguide treatment decisions and effectively documents member’s health status,functional status and diagnoses. As part of the Clinical Team, Nurse Practitionerswill be supported by a Nurse Practitioner Manager/Director and local MedicalDirectors, who provide education and guidance on best practices to effectivelycare for their members.
EssentialFunction- Provides collaborativeprimary care and services to a panel of Longevity Health Plan Services.
- Executes/deliversLongevity Health Plan Model of Care activities, treating in the mostappropriate setting develops and implements advanced care planning, embraces,and executes the treating in place model.
- Facilitates, administersHealth Plan clinical operations functions- prior authorization, Part A and Bservices, Medication Reviews, HEDIS, and STARs quality gap closure, promotingmembership growth.
- Serves as a trustedadvisor to members loved ones, and nursing home staff and member advocate,educates care teams on member needs and early identification of changes incondition, notifying NP and telehealth partners for member changes.
- Delivers effective andfrequent communication and coordination with members’ PCPs, loved ones, and careteam members om members care needs, goals of care, treatment plans/changes.
- Supports overall marketgoals/targets and balances member caseloads demands with other team membersupport needs.
- Promotes LongevityHealth Plan with members, loved ones and nursing facilities, and otherstakeholders.
- Is able to functionautonomously and is accountable for practice.
- Establishes meaningfulrelationships and is able to effectively function in the nursing homeenvironment.
- Is member-centric andcustomer service orientated establishing meaningful relationships that ensureseffective and high-quality care for Longevity Health Plan members.
- Provides support forother important duties as assigned.
Education- Masters’ of Science inNursing as a Nurse Practitioner required.
Experience- Minimum of one (1) to two (2) years ofexperience as a Nurse Practitioner OR one year of experience as a nursepractitioner if working in a long-term care setting AND has extensive experiencewith geriatric patients as a registered nurse prior to becoming an NP required.
- Experience working in the nursing homeenvironment is strongly desired.
- Computer/Technology proficiency required.
Licensing/Certification- State unrestrictedlicensed Advanced Practice Nurse with national AANP or ANCC required.
- Board certification asan Adult, Family, Geriatric or Primary Care nurse practitioner required. Willallow acute care nurse practitioner with nursing home/geriatric experience
- DEA License required.
- Must be eligible toparticipate in the Medicare program as a provider and registered.
- COVID 19 required
Knowledge/Skills/Abilities- The ability to engageand communicate effectively with physicians, patients, family members -excellent people skills.
- Self-directed but functionswell in a team environment.
- Excellent clinicalskills – assessment, diagnosis, treatment, prescribing specifically for thechronically ill population.
- Can functionindependently, demonstrated critical thinking skills, organization, andprioritization skills.
