The RN Care Coordinator utilizes a specialized knowledge of nursing and established practices and organizational policies to perform various duties to coordinate the care services for residents to ensure quality care for residents and provides direction to assigned staff. Since you are working with the same patients for long periods of time, you get the joy of building relationships with them. You’ll see your residents light up when you enter the room, help them build friendships, empower them to lead lives of purpose and be vital in ensuring we’re more than just a senior-living center, but a community!
Shift: Full-Time; 7:00am – 3:30pm; Monday - Friday
Job Description:
- Oversees documentation of all resident medical records to provide acceptable information for annual survey. Coordinates record keeping regulations regarding psychotropic drugs, restraints, resident rights, etc.
- Schedules appointments for consulting physicians, submitting transportation form as required. Prepares and sends necessary medical records with resident to appropriate physicians and/or hospitals. Participates in the QA process. Contacts resident's doctor, consultants, and/or family as necessary.
- Consults with specialists as required to provide special services and ensures resident care coordination.

Requirements:
- Valid Pennsylvania Registered Nurse license
- CPR certified
- 2-4 years nursing experience preferred
- Knowledge of HIPPA rules and regulations and of regulations and guidelines governing long-term care
- Strong managerial and communication skills to provide guidance and direction, set performance standards, monitor performance of subordinates and build trust among team members
- Ability to handle multiple responsibilities simultaneously, has ability to prioritize work and to address emergencies as they may occur
