Overview
As a Admissions-LPN you are responsible for facilitating resident admissions and discharges, and ensuring a seamless transition throughout their journey with us. You will work with our residents and their families as well as internal/external partners to ensure residents receive the highest quality of care and services.
Responcibilities
- The LPN conducts pre-admission assessment of prospective residents.
- The LPN develops and ensures continuity of dialogue with hospitals/social workers in community and outlying services regarding availability within community.
- The LPN will coordinate with social services department on discharge planning.
Qualifications & Skills
- Current state licensure as an LPN, in good standing
- Previous admission/discharge experience preferred
- Able to read, write, and communicate in English
