RN/LPN Unit Coordinator
Lockport Rehab & Health Care Center is actively seeking a Unit Coordinator. Lockport Rehab & Health Care Center is an 82 bed Sub Acute & Long Term Care facility. This Full Time position requires a self-directed, responsible individual who will oversee all aspects of resident care and assure continued delivery of service to our residents.
Responsibilities and Duties
- As a member of the Lockport Rehab team, this position requires critical thinking skills and the ability to demonstrate a level of proficiency in the management/oversight of their team.
- Mentoring of Nurses and Certified Nurses Assistants, interaction with families, departments such as Therapy, Dietary and Social Work to ensure comprehensive person centered care is delivered is an essential part of the position
- Assist with annual competencies and staff in-services
- Lockport Rehab offers Sub-Acute Rehab as well as Long Term Care
- 8 hour shifts / work and one weekend/month
Qualifications and Skills
- Current license in New York State
- Qualified candidates would be required to be flexible in their schedule

- Candidates with experience in dementia care, respiratory care, post operative care, wound care and end of life care will find satisfaction in working in this 5 Star building
