MDS Coordinator

We are looking for candidates with experience as an MDS Coordinator for this role

Our skilled nursing facility provides traditional nursing home care as well as sub-acute rehabilitation, hospice, respite, and Alzheimer’s care. Our goal is to achieve the highest practicable physical, mental, and psychological well-being while treating each resident with the utmost dignity and respect.

Requirements

  • MDS Coordinator: 1 year of experience required.
  • Must possess valid unencumbered, PA State License. LPN Nursing degree from an accredited college or university preferred.
  • Knowledge of Medicare and Medicaid regulations and coverage guidelines related to skilled nursing.
