Job description
Longmeadow of Taunton is a busy 89 bed skilled nursing facility seeking a full-time LSW, LCSW or LICSW. The position is Monday through Friday. We offer Competitive salary and benefits.
Qualifications:
- Master's degree preferred
- License as LSW, LCSW, or LICSW
- Knowledgeable of rules, regulations and guidelines that govern nursing facilities
- Communication skills with the ability to deal tactfully with personnel, residents, family members, visitors, and government agencies
Responsibilities
- Interview resident/family as necessary to obtain a social history
- Develop preliminary and comprehensive assessments of the social service needs of each resident
- Completing MDS and care plan timely
- Participate in discharge planning
- Attending facility required meetings
- Provide Medicare/Medicaid information to resident and family
- Completing Preadmission screening and resident review (PASRR) timely
- Monitoring resident services such as dental, podiatry, audiology, psych, etc.
- Assisting residents and families with grievances
Job Types: Full-time, Part-time
Salary: $53,160.00 - $76,960.00 per year
