Under the supervision of the Home Care Manager, the LPN is responsible for providing direct physical nursing care to the patient. This individual is responsible for providing the highest quality of patient care and ensuring patient safety at all times according to accepted nursing practice standards. The LPN acts as a primary support to the Physician, Nurse Practitioner, RN, Supervising Nurse or Director of Patient Services.
The practice of nursing as a Licensed Practical Nurse is defined as performing tasks and responsibilities within the framework of case finding, health teaching, health counseling, and provision of supportive and restorative care under the direction of a registered professional nurse or licensed or otherwise legally authorized physician or dentist (Education law, Article 139, Nursing, Section 6902).
Responsibilities:
Job duties specific to this position:
Provides quality care through observation and implementation of delegated tasks.
- Assumes responsibility and accountability and prioritizes care for all participants on the daily assignment, including providing direct physical nursing care and other duties as directed by the RN Case Manager.
- Performs participant care according to policies, procedure and standards.
- Observes the participant for specific signs/symptoms and reports changes in clinical and/or functional status and to the RN Case Manager in a timely manner.
- Ensures necessary supplies for participant care are available.
- Adheres to and promotes Participant Rights Regulations and confidentiality.
- Uses skills and knowledge according to the participant’s identified geriatric and age-related needs.
- Prepares, administers and records medications and treatments according to home care’s policies and procedures and in a timely manner.
- Possesses a sound knowledge base of medication administration.
- Provides the participant and/or family instruction on medication as it relates to the participant’s diagnosis, status or plan of treatment.
- Demonstrates appropriate knowledge base and technical skill when performing treatments.
- Provides education and training to participants and/or family members, observes and reports adherence to educational plan.
Contributes as a member of the Home Care Team.
- Collaborates with leadership staff in implementing planned changes and activities to improve the participant’s care or overall functioning of home care services.
- Attends and participates in Neighborhood and Home Care meetings as required.

- Demonstrates understanding and participates in the Home Care Quality Improvement (QI) program.
- Demonstrates appropriate performance to initiated changes in processes, based on QI results.
- Practices safety awareness, reports any possible safety/environment issues to supervisor and adheres to safety P&Ps
Provides leadership among co-workers.
- Adapts to changes in job responsibilities and accepts assignments in a professional manner.
- Participates in staff development programs to improve skills and to contribute toward the improvement of participant care.
- Demonstrates effective communication skills while working cooperatively with Home Care staff, members of the IDT and outside providers.

Documents in an appropriate, thorough and timely manner and in accordance with Loretto Geriatric Center Licensed Home Care Agency’s medical records policies and procedures.
- Collects and accurately documents nursing observations, interventions, and evaluations.
- Accurately transcribes physician orders under the supervision of the RN.
- Completes all service tickets for all care in a timely manner.
- Documents all medication administrations, pre-pours, wound care, vital signs, blood sugars, visits, phone calls, etc. appropriately and in a timely manner

- Must be a graduate of an accredited school of nursing for LPNs.
- Must possess a current NYS license and registration as a LPN.
- Minimum of one year experience as a LPN.
- Prior Home Care experience preferred.
- Able to function in a practice environment with minimal direct supervision, accepting
personal responsibility for maintaining a professional relationship with agency participants and their family.
- Excellent organizational skills.
- Excellent interpersonal skills.
- Proficient with MS Office and experience with Electronic Medical Record software. Ability to learn new software rapidly.
- Ability to be involved with family members or other support persons while providing care and interact professionally and respectfully.
- Ability to speak, read, read and comprehend English.
- Valid Driver’s License AND access to reliable transportation.
