What are the responsibilities and job description for the Support Coordinator position at Loving You Through It?
Support Coordination (SC) is a core specialty of our company supporting adults and children living with physical and mental disabilities. We need reliable and dedicated professionals to help us meet our mission and help our participants lead the independent lives they deserve.
If selected, you will:
- Complete home visits as required by funding source, generating required units (if applicable), observation and monitoring, and monthly/quarterly progress reports.
- Conducts and completes Assessments according to funder’s timelines.
- Planning, scheduling, and coordinating meetings with participants and providers.
- Responding to participant changes in needs, completing/submitting the appropriate paperwork.
- Researching, accessing, and linking services.
- Communicating and working with providers to ensure appropriate service provision.
- Maintaining all responsibilities and requirements in regard to critical incidents.
- Meeting with supervisor for supervision, consultation, and discussion of participant needs.
- Attending and participating in staff meetings.
- Attending and participating in required training.
- Making initial contact for new linkages.
- PROVIDES NEEDED ASSISTANCE OR FILLS IN FOR OTHER SUPPORT COORDINATORS AS NECESSARY AND/OR DIRECTED BY SUPERVISOR.

To be qualified, you must have:
-- a Bachelor's Degree in Social Services or Human Services related field. OR a degree with at least 16 hours in ONE area of Human Services (i.e. Sociology, OR Psychology, OR Education ,*OR Counseling, OR Social Services, OR Family & Participant Science, OR *Rehabilitation Services, or EQUIVALENT.).
-- Reliable transportation so that you may make visits to the field to conduct participant visits. These visits are generally held in the home of the participant. You may anticipate about 50% or more of your time will be in the field on home visits.
It is important to support coordination that employees understand that, barring accommodation, their commitment to our participant and the waiver program group includes participation in office schedules and good attendance.
