**This job has a benefit of a $3.00 per hour shift differential will be added for all hours worked on weekends or between 7pm and 7am**
Hourly Pay Range: $21.54-$26.92

As a Nurse, LPN you have the ability to direct ancillary staff/coordinate patient care during the shift.
Position Logistics
- 37.5-hour weekly one week, 50 hours the following week
- Routine Hours: 6:45pm-7:15am
- Overtime and flex schedule as approved by supervisor.
Minimum Education and/or Experience
Nursing degree from an accredited college or university; and one year of related experience and/or training are required. State LPN licensure and CPR certificate are required.
Position Overview
- Perform initial nursing admission assessments, to include Nursing History and Assessment, CIWA, COWS, nursing care plan and MAR records.
- Ensure appropriate medication regimen is provided to each patient per protocol and/or provider’s orders, providing education as needed.
- Monitor patient progress to assess for follow-up with the provider as needed.
- Complete initial screenings (phone and walk-in) and record information.
- Monitors patient progress, addresses patient needs, and maintains clinical documentation per established guidelines.
- Provides individual and group services, as directed, to address patient needs.
- Review completed screenings for medical eligibility approval.
- Prepare patients for discharge, to include Nursing Discharge Plan.
- Pre-certify and review patients with third party payers.
- Completes documentation per agency standards.
