I. TITLE: INVESTIGATIVE AGENT
CODE: MGT 565

II. SUMMARY:
An employee in this classification, under the supervision of the MUI Coordinator, is responsible for investigating Major Unusual Incidents (MUI) in accordance with applicable law and regulations, involving persons with developmental disabilities.

III. DUTIES:

50%
Conducts investigations of a variety of issues and personnel from a diverse group of service providers in accordance with standards of Ohio Administrative Code. Joint investigations also conducted with outside entities.
Identifies trends and patterns of Unusual Incidents (UIs) and Major Unusual Incidents (MUIs) and recommends review of individual service plan (ISP) for the purpose of improving and enhancing the quality and appropriateness of services.
Ensures adequate follow up has been taken by the provider/team to address the causes and contributing factors of MUIs and UIs.
Documents, prepares, and submits a report of the investigation on proper Ohio Department of
Developmental Disabilities (ODDD) form(s), and submits to ODDD within thirty (30) working days of the receipt of a report of a MUI.
Prepares and provides written summaries of MUI investigation to the individual, advocate, guardian and provider.
Notifies individual(s), advocate(s) selected by the individual(s), or the legal guardian(s) of the individual(s), and provider of right to submit written comments to the Board regarding the investigation's conclusion and any preventative measures implemented in response to the incident. Submits any written comments received to the ODDD and ensures adequate follow-up when department directed investigations are assigned.
Monitors activities to ensure compliance with all applicable rules, regulations, laws, policies/procedures, and collective bargaining agreements.

Participates in job related training and communicates with Board staff, ODDD, and Ohio Association of County Boards of DD to maintain a current base of knowledge and expertise regarding investigating MUIs.
Serves on related committees as assigned.
Acts as liaison and representative for ODDD in association with internal and external departments and agencies.
Acts as a witness in a variety of hearings.
Prepares a variety of reports, records, correspondence and other communication.
Evaluates, prepares and implements training of agency and county board providers related to the MUI rule.
Acts as a consultant to the county board and providers regarding criteria and adequate response to MUIs and UIs.

May be required to transport consumers and/or drive agency vehicle.
Maintains a variety of records, reports, correspondence and other communication.
Attends a variety of staffings, meetings, conferences, training and activities.
May be required to orient individuals assigned to the department.
Performs other related duties as required.

V. EQUIPMENT OPERATED: Personal computer, applications software, camera, and basic office equipment.

VI. WORKING CONDITIONS: Position may involve exposure to individuals with medical and behavioral risks.

VIII. MINIMUM QUALIFICATIONS, EDUCATION AND EXPERIENCES:

A. Education:
Bachelor degree required.

B. Experience:
Two (2) years of experience with conducting interviews, gathering and analyzing data, conducting assessments, and writing reports. Experience providing service coordination for or working with individuals with DD preferred.

Licensure:
Valid driver license.

D. Ancillary Training:
Community First Aid/CPR
Specialized Support Resource Book Training

E. Other:
Will require evenings and weekends, and locations convenient for person(s)/family(ies) served.
Must be qualified to perform all duties assigned to this position.
