MDS / PDPM / Case Management Coordinator

Lutheran Home of Southbury is on the look out for an experienced MDS/PDPM Nurse.

Previous experience under the new guidance that was effective back in October of 2023 is a MUST.

Essential Responsibilities

• Chair IDT Resident conferences for each level of care; schedule conferences according to mandated time frames, both OBRA & PDPM.
• Provide Case Managment related to managed care.
• Maintain a written interdisciplinary care plan for all residents. Acts as interdisciplinary liaison in developing the most appropriate plan of care.
• Attends other family meetings and or conferences as needed. Acts as liaison between family and facility using excellent communication and written skills.
• Participates in the organizational CQI committee, identifies process improvements needs related to Resident Care assessment/care planning process.
• Coordinates and oversees Assessment Date Entry/Transmission maintaining date accuracy and integrity.
• Provides excellent customer service to all who interact with the program

Qualifications

• Associates or Bachelor’s Degree in Nursing or related field preferred.
• Must possess a current and valid Connecticut RN license or LPN Connecticut license.
• Must have, as a minimum, one (1) year of experience with Resident Care Planning and MDS and Medicare/PPS
• RAC-C preferred

Starting at $35.00 per hour. Salary will be based on experience.
