Lutheran Life Villages is in search of a SOCIAL WORKER for our Anthony campus! Are you a Social Worker or Social Services Designee looking for an opportunity to make an impact every day?
As a representative of the Social Services department, you will be monitoring the overall health and well-being of our residents through the use of assessments, care plans, and Interdisciplinary Team meetings. The Social Worker will be getting to know residents on a personal level and conduct routine BIMS and PHQ-9 screenings, to assess cognition and emotional wellness. Through the use of assessments and guidance of mental health practitioners, you will be a big part in advocating for the residents' well-being. You will also get to be an advocate for our residents' rights and supporting their dignity every day.
Schedule is Monday - Friday Dayshift!

Job Qualifications
Education/Training:
- Social Service Bachelor's Degree (BSW) preferred
- Bachelor degrees identified within the CMS/ISDH regulations are also acceptable; or
- Social Services Designee
Experience:
- Minimum one (1) year experience working with residents in a long term care setting.
Required Knowledge, Skills, and Abilities:
- Ability to create and manipulate documents using Microsoft Word, Excel, and PowerPoint.
- Working knowledge of long term care rules and regulations.
- Ability to plan, direct, coordinate, and implement overall care functions.
- Ability to communicate effectively in English.
- Excellent proven interpersonal, written, verbal and organization skills.
