We’re currently seeking a dedicated Registered Nurse Assessment Coordinator (RNAC) to help lead our nursing team at Spang Crest, a Luthercare community delivering skilled nursing care and rehabilitation services in Lebanon, PA. This is an exempt-level position.

WHAT YOU DO: 

As the Registered Nurse Assessment Coordinator (RNAC) at the Spang Crest Healthcare Center, you will be a key leader on our nursing team. In the role, you will coordinate and provide resident assessments, ensuring the highest of standards in accordance with current federal, state, and local guidelines and regulations.

You will perform a range of duties, including:

• Coordinate and maintain all required schedules for MDS/Resident Assessment Instrument (RAI) for residents upon admission, quarterly, annually, with condition change and following current CMS Medicare/Managed Care schedules. Provide in-service education in the use of the RAI.
• Coordinate assignment of support documentation for RAI to all departments. Assess changes in resident condition daily and coordinate new RAI processes when applicable.
• Coordinate all applicable support documents, including codes, batch checks, MDS printing as needed, and locking procedure for RAI within time frames and in accordance with state and federal regulations.
• Review and assess potential new admissions as needed prior to admission and provide information to applicable staff. Document resident information scheduled for discharge in accordance with regulations.
• Coordinate care plan schedules/standards and ensure compliance through education/review/audits.
• Coordinate Medicare utilization coverage with all applicable departments as required. Facilitate weekly review meetings.
• Provide RN in-house coverage as needed. Transmit state-required incident reports as needed.
• Coordinate the restorative program. Perform resident documentation as required following Luthercare’s Medical Record Documentation standard. Perform nursing duties following licensure scope of practice.

The ideal candidate for this position is a PA Registered Nurse with 3 to 5 years’ clinical experience in a long-term healthcare center and who has demonstrated knowledge of federal/state regulations governing nursing services and medical records standards.

Spang Crest is not a standalone skilled nursing facility; it is part of Luthercare, which is a not-for-profit, faith-based organization in southcentral Pennsylvania.  QUALIFICATIONS/REQUIREMENTS: 

• Current Licensure as a PA Registered Nurse.
• Current cardiopulmonary resuscitation (CPR) certificate.

ESSENTIAL JOB-RELATED EXPERIENCE:

• Three to five years directly related experience in a long-term healthcare center.
• Demonstrated knowledge of the laws and regulations regarding MDS, Medicare, Managed Care, Medicaid, case mix, current RUGS, and the resident classification system.
• Ability to lift and turn residents with a maximum of 50 lbs. effort and perform the functions required to push residents in wheelchairs.
