Social Service Director Job Specification:
The Social Service Director will play a vital role in advocating for the social and emotional well-being of our residents and coordinating social services to meet their needs.
Social Service Director Essential Duties & Responsibilities:
- Assess the psychosocial needs of residents upon admission and develop individualized care plans in collaboration with the interdisciplinary team.
- Provide counseling and emotional support to residents and their families, addressing issues such as adjustment to the facility, coping with illness, and end-of-life care planning.
- Coordinate with community resources and agencies to arrange services such as transportation, financial assistance, legal aid, and counseling for residents and their families.
- Advocate for residents' rights and ensure that their preferences and concerns are addressed in care planning and decision-making processes.
- Collaborate with healthcare professionals, including physicians, nurses, and therapists, to address the social and emotional aspects of resident care.
- Maintain accurate and confidential resident records, documenting social service assessments, interventions, and outcomes.
- Participate in interdisciplinary team meetings, care conferences, and quality improvement initiatives.
Social Service Director, Here’s What You’ll Need::
- Bachelor's or Master's degree in Social Work or a related field from an accredited institution.
- Current licensure or certification as a Social Worker in the state of Kentucky.
- Previous experience in a long-term care or healthcare setting preferred.
