Community Support Coordinator
(Housing and Core Services )
Full-time (37.5 hrs/wk) Community Support Coordinator for busy, non-profit, cross disability services agency. BA/BS in human services or related field and 2 years’ experience in disability-related services preferred. Assist consumers with all types of disabilities who have low income, are near homeless or homeless, and need assistance to find or keep appropriate housing and to live independently in the community. Provide consumers with advocacy, independent living skills, peer support and information & referral, and conduct outreach. Knowledge of community resources for persons with disabilities a plus. Highly organized individual with commitment to consumer control and empowerment sought. Ability to work in a team environment. Strong communication, customer service and Microsoft Office skills required. Some local travel required. Essential Job Duties:
Meet with consumers for intake and CDBG income verification at LVCIL offices, community locations, at consumer homes throughout the Lehigh Valley or through virtual platforms as needed.
Educate consumers on housing options.
Assist homeless, near homeless and other consumers with search for housing including Section 8 applications, apartment applications, contacts to private landlords, and contacts to housing authorities.
Advocate for housing placements to occur and/or to help consumers retain appropriate housing. Provide case management to consumers throughout housing transition. Provide follow-up contacts to consumers.
Make preliminary visits and contacts to housing options on behalf of consumers.
Provide ongoing support to consumers; facilitate referrals to other services as needed. Coordinate consumer services with Core & Housing or other LVCIL team members.
Provide Independent Living Core Services as needed to consumers including advocacy, peer support, independent living skills education and information & referral. Provide services on an individual or group basis as needed. Support LVCIL consumer groups.
Provide outreach contacts to community organizations; participate in related community groups.
Provide landlord tenant mediation as needed.
Work with landlords to increase opportunities for persons with disabilities.
Record consumer contacts and all time use in web-based agency database systems on a daily basis.
Complete reporting as requested. Prepare monthly time certifications and other reporting as needed.
Function as part of LVCIL Team and Core & Housing Team and attend related meetings.
One of the two positions will have more of a focus on working with Veterans with disabilities, especially related to housing and employment needs.
Hiring for two full time positions
$18 per hour
