Overview
Fairview Range is hiring a Community Health Worker for our Behavioral Health Home Department in Hibbing, MN.
The Community Health Worker (CHW) is responsible for helping community members, patients, and their familys access and navigate community services and resources and adopt healthy behaviors. The CHW supports providers and team members through a coordinated approach to care management and community outreach. Activity will promote, maintain and improve individual and community health. The CHW provides social support and informal counseling, individual and group health education and advocacy for individuals and community health needs. Providing home visits and health screenings may be required.
Schedule: This position will work 72 to 80 hours per two weeks. The shift length will be 8-hours on the day shift. Weekends: No weekends

Responsibilities/Job Description
As a Community Health Worker, you will be responsible for:
Promoting wellness by providing culturally appropriate health information to providers, patients, and/or community members.
Providing individual social and health care support.
Growing access to primary care through culturally proficient outreach and enrollment strategies.
Making referrals and coordinating services.
Advocating for individual and community needs.
Bridging the gap between communities and the health and social service systems.
Qualifications
Required Qualifications
Community Health Worker Certificate through an accredited program, or Associates Degree in Health, Human Services, or related field.
3 years experience working on community health projects, programs, or initiatives.
Community based leadership experience.
BLS
Preferred Qualifications
Bachelors degree in health, Human Services, or related field.
Experience teaching or presenting information to groups.
Experience with Microsoft Office.
Multicultural healthcare experience or healthcare experience in other countries.
