Job Description:
The service navigator will oversee the Community for Life program, recruit members and provide service navigation, transportation assistance, and home maintenance oversight for clients as well as marketing program and maintaining financial records pertaining to membership.
The position reports directly to the Director of Community for Life, Wicomico County.

Essential Responsibilities:
· Fluent with Microsoft Office.
· Obtain referrals for new members as well as recruit new members and maintain an active membership roster in program.
· Work with Senior Care and house subsidy staff with wait list.
· Upon completion of initial membership application, complete Home Maintenance and Safety Assessment for new members
· Make weekly check in calls to members.
· Arrange monthly transports, maintain database of all transports, and coordinate financial reporting with Comptroller.
· Make bi-weekly home visits to members, helping in the home.
· Maintain all documentation related to the program including client files and referral data.
· Provide reports as needed on client outcomes, number of clients served, demographic data, etc.
· Assure participant’s personal information is handled in accordance with privacy statements and policies.
Qualifications:
Bachelor’s Degree with minimum 2 years’ experience working with senior citizens.
OR
Associate degree with minimum of 5 years’ experience working with senior citizens.
