About the position:
The inpatient Case Manager is responsible for assessing, planning, implementing, coordinating, monitoring and evaluating the needs of patients and healthcare services delivered across the continuum of care to ensure quality patient outcomes and appropriate utilization of health care services. The case manager works in conjunction with Social Services and is responsible for the coordination of discharge planning of all patients with physicians, nursing staff, patient and significant others who have an ongoing caring relationship with the patient in order to provide the best continuity of care for the patient at the time of discharge.
Key job qualities/duties:
Performs ongoing assessment of patient condition to determine needs of the patient through proactive care facilitation and discharge planning. Documents assessments and discharge plans in an ongoing manner in the hospital established case management data base software. Collaborates with the physician, other health care team members, patients and families pertaining to all potential discharge planning needs, and facilitates post-discharge care.- Addresses and supports cultural practices, as long as such practices do not harm others or interfere with the planned course of medical treatment.
- Assists patients with completion of advance directives, DNR or POA
- Attends daily unit meetings to discuss patient progress, plans for continued care and discharge needs. Work effectively with physicians in order to provide continuity of care for patients. Strives for excellence in clinical practice, patient/family teaching and communication skills.
- Completes a psychosocial assessment of patients and families. Identifies the patient’s support system and coping mechanisms
What qualifications are we looking for?
Graduate of an accredited nursing school with current license to practice as a Registered Nurse in the State of Ohio- BLS certification within 30 days of employment
- Five (5) years direct patient care experience required. Recent experience with Medicare/Medicaid and Commercial Insurance preferred.
- Basic computer skills required; prior EPIC patient charting experience preferred
