We are looking for compassionate and talented team members who share our vision. The Care Manager supports the health and well-being of our clients through the provision of case management and counseling services.
Essential Responsibilities of the Care Manager
- Provide case management services
- Complete therapeutic assessments and work with each person to develop person-centered recovery goals
- Counsel clients, including crisis counseling and intervention
- May be required to run therapeutic skills groups
- Facilitate communication with client’s families, guardians, and parole and probation officers
- Facilitate linkage between client and community agencies
- Advocate for client rights and well-being.
Qualifications for the Care Manager
- Bachelor’s or master’s degree in social work, psychology, or related discipline
- 1-3 years’ mental health experience preferred
