RN Case Manager
Job Posting for RN Case Manager at Madrone Hospice, Inc
Assess patient/family needs with the patient's attending physician, develop, review and revise the patient’s plan of care, provide skilled nursing and supportive care in accordance with the attending physician's orders and plan of care, meet with Interdisciplinary Team weekly to reassess plan of care.
Job Type: Full-time
Pay: $33.00 - $43.00 per hour
Expected hours: 32 per week
