➢ Resident Placement and Care:
- Identify and recruit new residents
- Coordinate a transfer of new resident in suitable residential care settings that meet individual needs and preferences.
- Develop and Implement individualized PCP service plans, regularly updating goals and update resident progress.
➢ Operational Oversight:
- Oversee daily operations, ensuring a safe and healthy environment for residents and staff.
- Participate in residents’ meetings to discuss care plans, progress, and concerns.
- Conduct internal investigations and address any issues related to resident care or staff performance.
➢ Communication and Coordination:
- Maintain regular and effective communication with guardians, case managers, and other relevant stakeholders to coordinate care and address any concerns.
- Facilitate residents’ meetings to review and discuss care plans, progress, and any issues that arise.
➢ Documentation and Reporting:
- Enter General Event Reports (GER) into DHHS database accurately and promptly
- Maintain comprehensive and accurate records of residents’ care, staff activities, and facility operations.
- Oversee the completion and signing of all annual paperwork and consents.
➢ Staff Management and development:
- Assist residents and staff with developing daily schedules that include resident interests and community outings.
- Conduct clinical meetings for staff to discuss resident care, address concerns, and provide training as needed
- Offer feedback and recommendations for improving staff performance and operational efficiency.
➢ Regulatory Compliance:
- Ensure that all facility operations adhere to relevant regulations, standards, and policies.
- Implement changes and improvements to enhance the quality of care and operational effectiveness.
- Manage clinical books and ensure all necessary documents are up-to-date and properly maintained.
- Compassionate, detail-oriented, and able to handle stressful situations with professionalism.
Qualifications:
- Education:
- Bachelor’s degree in social work, Psychology, Nursing, or a related field. Advanced degrees are preferred.
- Relevant certifications and licenses as required by the State of Maine regulations.
➢ Experience:
- Minimum of 5 years of experience in a clinical or residential care setting, including at least 2 years in a managerial or supervisory role.
- Proven experience in managing operations and developing individualized care plans.
