  • Eligible candidates are hired (offer accepted) into a Full or Part time Case Managers position. Bonus amount prorated for Part time hires, per diem hires are ineligible.

~ 40 Hours/week, Hourly, Monday – Friday, Day shift ~

Maine state licensure to practice as a Licensed Social Worker (LSW) preferred. Multi-lingual preferred.

 

Position Summary

Located at the pediatric clinic in Maine Medical Center.      

The ECSS Case Manager is responsible for addressing social determinants of health (SDOH) and other risk factors that impede healthy development.  The position will focus primarily on children ages 0-5 and includes parent coaching and team consultation, with the goal of promoting early child mental health, building resilience in the parent/child relationships, as well as accomplishing goals identified by each family.  Establishing connection to the mother in women’s health prior to birth of child is part of the work. The role will ensure that service delivery practices are ethical, culturally competent, trauma informed, respectful of diversity and inclusion, welcoming and geared to custom service satisfaction.  The ECSS Case Manager will be compliant with regulatory, accreditation, contract and best practice standards. ECSS activities will be performed primarily in the assigned pediatric clinic and via remote connection as project teams determine based on needs and social distancing guidelines.   Home/community visits are also required.

Required Minimum Knowledge, Skills, and Abilities (KSAs)

  1. Education: Bachelor’s degree from an accredited institution with a specialization in early childhood development/education, psychology, behavioral health, social work, special education, or closely related field is required.
  2. License/Certifications: Eligibility for LSW licensure preferred. Valid Driver’s license and appropriate level of automobile liability insurance.
  3. Experience: 1 year of experience in the field of early childhood development. 2 years of experience working with children and families within a broad spectrum of cultural and socioeconomic backgrounds is required. Prior experience with parenting education, coaching is preferred.
  4. Excellent computer skills are required: Microsoft Word, Excel and use of electronic consumer record software
  5. Knowledge and commitment to abide by the Code of Ethics of the National Association of Social Workers
