What are the responsibilities and job description for the LPN / MEDICAL LIAISON position at Mary Cariola Children's Center?

Follows the responsibilities, as necessary, of a mandated reporter.
Remain alert and responsive while on scheduled shift(s).
Administers medications to residents in accordance with the Residential Medication policies and procedures.
In conjunction with the Residential RN, schedules medical, dental and lab appointments according to need. Attends appointments and/or assures that appropriate staff is scheduled to transport children on appointments, in conjunction with the Residence Manager. Notifies parents/clinicians and day programs of upcoming appointments.
Maintains adequate supplies of medications and non-prescription items for residence and day programs. Orders medications and supplies, as necessary. Picks up medications and supplies, as needed, if they are not delivered by the pharmacy. Ensures the medications and supplies are stored in an appropriate manner according to regulations.
Performs weekly checks in the Med Room, including but not limited to, checking expiration dates on medications, topical treatments, etc. and discarding all expired medications and supplies, as per written procedures; maintianing the organization of the med room; checking the empty medication bottles and bubble packs and notifying the RN of any errors found; and working with the RN to ensure all posted nursing notes are visible and up-to-date.
Assigns AMAP for each shift and confirms that there is at least one med-certified staff available to each shift. Assures that every AMAP passes meds no less than twice a month.
Maintains CPR-SFA-PPD records of staff.
In conjunction with the Residential RN, provide orientation of new AMAP staff to the med room and med training activities.
Checks first aid kits in vans and residence, restocks PRN and signs off with date.
Checks consumer specific med info sheets and ready-to-go packets and signs off with date.
Ensures vital signs and heights/weights are obtained as ordered.
Is responsible for accurately and consistently completing all required documentation practices and assigned paperwork.
Provides a level of oversight to the DSP staff, during assigned direct care shifts, to ensure that the shift responsibilities are completed. Promotes a team-oriented atmosphere. Actively participates in the shift to model appropriate behavior and professional work habits.
Professionally addresses observed staff conflict or performance issues in real time, during assigned direct care shifts. Regularly informs the Residence Manager of personnel problems and contributes recommendations for resolution. Communicates all necessary and important information relative to the individuals to the Residence Manager.
Facilitates active participation of the residents by implementing their individual program plan in their home and out in their community. Completion of this responsibility occurs while working closely with fellow staff, as well as, working one-on-one with a resident in an isolated environment within the house or out in the community. These responsibilities include, but are not limited to, engaging in necessary lifting, moving and positioning tasks; facilitating improved gross motor, fine motor and range of motion skills; promoting self care, activities of daily living and personal hygiene; fostering receptive and expressive communication through many different avenues such as, sign language, pictograms, electronic devices, etc; assisting with meals through modeling appropriate behavior, therapeutic feeding and use of therapeutic equipment; assisting with recreational activities and facilitating active engagement in community outings; accurate and consistent implementation of behavior support plans/SCIPr.
Facilitates and/or provides for the personal and physical care needed by each individual including tasks such as bathing, toileting, eating and dressing.
Performs housekeeping tasks as designated by the shift supervisor/Residence Coordinator including tasks such as meal preparation, cleaning, laundry and shopping.
In keeping with confidentiality and HIPAA practices, communicates a wide range of information regarding the residents to a variety of people in and out of the residential program such as co-workers, teachers, supervisors and parents.
Attends and actively participate in, at least, biweekly staff meetings as scheduled or requested by the Residence Manager.
Is responsible for following the Code of Ethics for Direct Support Professionals, as well as, show proficiency in the Core Competencies to ensure person-centered supports for the residents.
Attends and actively participates in case reviews, in-service sessions, team meetings and other meetings upon request.
Implements fire, safety and emergency procedures and assists with fire drills.
Implements and follows all required individual programming, Agency and Program-specific policies and procedures accurately, consistently and according to how they are written.
Safely drives Agency vehicles. Assists in transportation to and from recreational activities and other appointments as required.
Maintains Agency-Approved Driver status, as well as, certifications in Medication Administration, SCIP-r, CPR and First Aid. Completes all required ongoing/annual trainings and inservices, as assigned.
May be assigned additional responsibilities by the Residence Manager and/or Residential RN.
Other duties may be assigned.
