What are the responsibilities and job description for the Social Worker position at Maryland Medical Day Services II?
1. Coordinate and implement individual, group, and family counseling in conjunction with plan of care goals.
2. Inform participants and their families of available community services and to refer participants, as necessary, to agencies providing these services.
3. Write monthly progress notes in participant’s record.
4. Be responsible for documenting the follow up of all absences.
5. Complete an admission social work assessment and mini mental status exam within 14 days of participant’s admission.
6. Complete monthly ADCAP assessment.
7. Coordinate Psychiatric F/U and schedule initial therapist evaluation and Psychiatrist evaluation.
8. Complete POC for every problem client exhibits and evaluates them every 90 days.
9. Coordinate discharge planning and refer participant to outside agencies.
10. Reports any and all suspicions of abuse/neglect to APS and documents accordingly.
11. Communicates with Care Provider any and all concerns or changes in POC.
12. Coordinates Care conferences with Care Providers and interdisciplinary team.
13. Coordinates referrals to outside community resources as needed.
14. Marketing and oversee admissions.
15. Documents on all of participant housing changes in record and follows up accordingly.
16. Coordinates admission process and receives all needed information form referring agency.
17. All other duties assigned by supervisor.
