We are looking for a Registered Nurse to join our home health team. The ideal candidate will have experience providing direct patient care, as well as the ability to work independently and collaboratively with other healthcare professionals. The successful candidate will be responsible for providing assessments, treatments, and patient education in the home setting. This is an excellent opportunity for a Registered Nurse to make a positive impact on patients' lives and provide quality care.
Responsibilities:
- Provide direct patient care in the home setting.
- Assess and evaluate patient needs and develop individualized care plans.
- Provide education to patients and families regarding health maintenance, disease process, and self-care skills.
- Document patient care in accordance with professional standards and regulations.
- Monitor patient response to treatments and medications.
- Coordinate with other health care providers to ensure continuity of care.
