The Occupational Therapist is responsible for performingclient evaluations, developing and providing occupational therapy services, anddocumenting services in accordance with the plan of care developed for eachpatient.
Essential Duties and Responsibilities:
- Provides skilled occupational therapy services/interventionsin accordance with physician orders
- Reevaluates client for rehabilitation needs, and works incollaboration with the physician and other members of the healthcare team tomodify the plan of care as appropriate
- Assesses patient needs, plans for, evaluates and modifiescare to meet goals of Occupational Therapy Interventions
- Collaborates with all disciplines to plan and evaluate teamgoals for each patient
- Instructs patient/family/caregivers on patient's totaloccupational therapy program
- Advises, consults and instructs members of themulti-disciplinary team in specific occupational therapy modalities
- Observes and reports patient progress and/or changes inpatient status to physician and appropriate multi-disciplinary team members
- Refers individual to other disciplines as deemed appropriateand as approved by the physician
- Teaches new skills or retrains clients in once-familiardaily activities that have become difficult or unfamiliar due to illness orinjury
- Establishes equipment needs to facilitate independent living
- Instructs individual and/or family/caregiver in the use ofvarious types of adaptive equipment to improve function ensures equipment isin good working order
- Trains individual and/or family/caregiver in use ofprosthetic and/or orthotic devices
- Performs work capacity evaluations and may refer individualto specialized vocational training in accordance with policy and regulations
- Provides accurate and timely documentation according to theplan of care
- Participates in care conferences or other meetings asrequired to ensure coordination of home health services (if applicable)
- Prepares and submits clinical progress-summary based on theprogress of goals
- Provides in-services/education programs for variousdisciplines as needed
- Participates in peer consultation process
- Performs other duties as assigned/necessary
Minimum Requirements:
- Current licensure as an Occupational Therapist in the Stateof practice
- Must be a graduate of an accredited school or program ofOccupational Therapy
- One (1) year of prior professional Occupational Therapyexperience preferred
- Complies with all relevant professional standards ofpractice
- Participation and completion of Maxim’s Competency programwhen applicable
- Current CPR if applicable
- TB Questionnaire, PPD or chest x-ray if applicable
- Current Health certificate (per contract or stateregulation)
