General Summary:
Develops and implements Supportive Living Services (“SLS”)/Independent Living Services (“ILS”) programs, policies, and procedures to provide the necessary support for each Individual to be fully integrated into the community while maintaining their home, health, and safety. This position provides effective case management and coordination for each assigned Individual daily by overseeing a team of Instructors, Field Trainers, Field Supervisors and Personal Assistants.
Essential Functions:
- Manages a team of Instructors, Field Trainers, Field Supervisors, and Personal Assistants to provide support and supervision of Developmentally Disabled Individuals and Individuals with other medical diagnoses that impair their cognitive abilities
- Selects, trains, and manages an interdisciplinary team that support and supervise assigned Individuals to their desired goals reflected on their Individual Program Plans “IPPs”
- Manages labor hours including the scheduling of meal and rest periods and time record approvals
- Applies best practices regarding the rights of Individuals with developmental disabilities, family dynamics, and Individual’s preference of support
- Reviews the referral packets of all current and newly assigned Individuals to develop appropriate service plans
- Utilizes a person-centered approach to collaboratively determine what is "important to" and "important for" the Individual
- Implements and executes Individual Program Plans (“IPPs”) and Individual Support Plan (“ISPs”) and monitors each Individual’s progress
- Supervises IPP/ISP meetings with the interdisciplinary team to assess and reach the desired goals of Individuals
- Gathers needed documentation (including case notes and doctors’ notes) to create structured reports for 30 day, quarterly, and ISPs for timely submission
- Initiates and completes Special Incident Reports (“SIRs”), tracks patterns, and develops preventative action plan through risk management and assessment
- Organizes and manages staff schedules for the successful delivery of services to Individuals.
- Oversees service hours for assigned Individuals to ensure they are being provided and accurately recorded for proper submission every month
- Supports and executes through training and monitoring of best practices when staff transfer and reposition non-ambulatory Individuals safely
- Trains staff and monitors the protection of Individual's rights and Title 17
- Supports and implements an Individual’s wish to change or amend their ISP or IPP
- Monitors the comprehensive approach regarding an Individual's medical history and barriers and how to provide support to achieve their desired outcomes
- Manages and coordinates the Individual's interdisciplinary team to provide accurate input of individual performance and progress
- Establishes and maintains open lines of communication between Individuals and their circle of support, staff, and regulatory agencies
- Builds and maintains professional working relationships with Regional Centers to strengthen company rapport
- Complies with state and local guidance regarding the assistance of prescription medication administration for Individuals
- Schedules, reviews, and coordinates training content to adhere with state requirements for interdisciplinary team
- Coordinates and ensures observance of Individuals’ appointments, court dates, inter-agency involvement, volunteer hours, work schedules, etc.
- Maintains and manages employee training and evaluation tracker
- Attends all company-mandated trainings required for the position
- Performs all other duties and responsibilities as assigned
Qualifications:
- Minimum High School graduate or GED equivalent required
- AA Degree in Behavioral Science or closely related field required or corresponding related experience
- Bachelor's Degree in Psychology, Special Education, or closely related field preferred, but not required.
- One year of recent full-time experience working with Individuals that have intellectual or developmentally disabled diagnoses, or Alzheimer’s disease, or other severe mental/behavioral challenges in a hospital or nursing home setting required
- Two years of recent full-time experience working with individuals that have intellectual, developmental disabilities, or severe behavioral challenges, or dual diagnoses preferred
- Certified in Crisis Prevention Intervention (CPI) or must successfully complete in-house program
- Must have a current First Aid and CRP card or must successfully complete in-house program
- Complete a minimum of 20 hours of continuing education units annually
- Successful completion of Support Professional I and II certification which will be provided after hire
- Ability to speak, read, and write in English is required. The ability to speak Spanish is highly desired.
