Health Care Case Manager

We are currently looking for an experienced Case Manager near Champaign County, Illinois. You will be servicing Champaign County and the surrounding southern counties.

This position is mostly REMOTE but you will occasionally complete face to face assessments.

You must be located near Champaign County.

This position is a 6 month contract with a possibility of an extension.

SHIFT:

• 8:30 AM - 5 PM CST
• Monday through Friday

Experience:

• 2 years of Case Management experience is required
• Must be comfortable working with healthcare software

Requirements:

• Active, RN, LSW, LCSW, LPC or LCPC license
• Must live near Champaign County

The Case Manager is responsible for health care management and coordination of healthcare members in order to achieve optimal clinical, financial and quality of life outcomes. Works with members to create and implement an integrated collaborative plan of care. Coordinates and monitors member's progress and services to ensure consistent cost effective care that complies with company policy and all state and federal regulations and guidelines.

• Carries a Case Management caseload of approximately 115 LTC (Long Term Care) Cases
• Will complete face to face visits with members in the LTC Facilities at a minimum of an annual visit per member
• Will complete an annual assessment, Assessment Summary (DAR), and Care Plan per member
• Will collaborate with the LTC provider staff
• Participates as needed in Clinical Care Rounds
• Participate in auditing both internally and externally

Job Types: Full-time, Contract
