The LPN, Staff Nurse in the Medford Leas Wellness Center is responsible for providing and coordinating all nursing care for his/her resident based on established clinical nursing practice standards. Collaborates with other professional disciplines to ensure effective and efficient resident care delivery and achievement of desired resident goals.
This job allows you to practice what you love about nursing, hands-on patient care that extends from the office into the home. Daily interactions/collaborating with the NP/MD about the care of each patient in a friendly, professional atmosphere. Coordinates own workflow for resident care needs. On-call for and responds to all in-house/on campus emergencies.
Weekend Days (Saturday/Sunday) = 12 hour shift each day
Monday = 8 hour shift

Core Responsibilities:
- Under the direction and guidance of the Nurse Practitioner/MD/DO, administers general nursing care to residents, including treatments and medications, as assigned and in accordance with the Standard of Practice for the Licensed Practical Nurse in the State of New Jersey
- Participates with the Nurse Practitioner/MD in planning, directing, and coordinating resident care, staff assignments and supervision. In the absence of the Nurse Practitioner/MD, oversees performance and behaviors of clinical staff to ensure that all policies and procedures and clinical standards are upheld. Reports any violation of policy or procedure to the Nurse Practitioner/MD.
- Maintain the confidentiality of all resident care information including protected health information. Report known or suspected incidents of unauthorized disclosure of such information. Adhere to policies and procedures.
- Ensures residents are knowledgeable of their’ responsibilities and rights including the right to refuse treatment.
- Ensures staff respect and uphold resident rights
- Report complaints and concerns made by the resident and make a written/oral report to the Nurse Practitioner/MD indicating what action(s) were taken to address and resolve. Reports unresolved complaint or concerns to the Nurse Practitioner/MD. Follow the facility’s established procedures.
- Effectively communicates resident needs or change in status with practitioners by preparing an appropriate report using the SBAR or similar format
- Executes physicians’ orders with an understanding of why the orders were issued and with a knowledge of their desired or possible adverse effects; assists the physician in obtaining accurate medical histories and with performing certain procedures; in all cases, completes required documentation relative to the execution of all orders and procedures; completes nursing notes and all other documentation as required accurately and timely
- Involves residents in their plan of care; interprets the needs of residents to appropriate personnel
- Records and reports pertinent observations of residents’ responses to medical & nursing therapy according to procedure.
- Perform other duties as assigned or directed
Education/Experience Requirements
- Graduate of an accredited School of Nursing
- Must possess excellent time management, organizational, planning and multitasking skills
- Must be able to hear, read, write, speak English
- Must be able to use a calculator, and perform math functions such as addition, subtraction, multiplication, division, percentages for drug calculations
- Computer literacy
License/Regulatory Requirements
- Current, unencumbered State of New Jersey LPN license
- CPR/AED certified
- Must obtain all required annual industry related / regulatory immunizations (including completing the annual TB Assessment) and provide documentation of same
- Must be free from communicable disease as required by the Department of Health
Physical Requirements/Environmental Conditions
- Must be able to lift up to 35 pounds
- For up to 85% of the time, must be able to read, write, speak, hear, lift/assist residents into and out of beds, chairs, and the bathroom as well as turn residents in bed and engage in other physical activity, including bending, stooping, carrying, and standing, sitting, or walking for prolonged periods
- Involved with residents, personnel, visitors, government agencies/personnel, etc., under all conditions and circumstances
- Subject to hostile and emotionally upset residents, family members, personnel, and visitors
- Communicates with the medical staff, nursing personnel, and other department directors.
- Works beyond normal working hours, and in other positions temporarily, when necessary
- Subject to call back during emergency conditions (e.g., severe weather, evacuation, post-disaster, etc.).
- Subject to injury from falls, burns from equipment, odors, etc., throughout the workday, as well as to reactions from dust, disinfectants, and other air contaminants
- Subject to exposure to infectious waste, disease, conditions, etc., including TB and HIV, MRSA, C-Difficile and Hepatitis B viruses
- May be subject to the handling of and exposure to hazardous chemicals.
