Job Description: The Admission Nurse Evaluator plays a vital role in facilitating the admissions process for our skilled nursing facility. The Admission Nurse Evaluator is responsible for clinically reviewing prospects, ensuring a seamless transition into our facility.

Responsibilities of Admission Nurse Evaluator:

  • Conduct initial screenings and assessments of prospective residents to determine eligibility.
  • Reviews PRIs and all supporting clinical documentation for appropriateness to the facility.
  • Maintain accurate and up to records of all admissions-related activities, ensuring compliance with relevant regulations and document requirements.
  • Collaborate with healthcare professionals to obtain the necessary documentation for admissions.
  • Review/Analyze census reports for discharges, admissions, hospitalizations, and readmissions with follow-up as necessary.
  • Coordinate with the facility to ensure adequate preparation for new admissions including special accommodations.

Qualifications for Admission Nurse Evaluator:

  • Graduate of an accredited school of nursing.
  • Active NY State of Board of Nursing License and Registration.
  • Preferred prior Skilled Nursing Facilities experience.
  • Knowledge of Healthcare and Hospital systems, especially in Suffolk County area.
  • Strong presentation and negotiation skills.
  • Excellent communication and interpersonal skills, with the ability to build and maintain strong relationships.
  • Results-oriented mindset with a proven track record of achieving targets.
  • Self- motivated, proactive, and able to work well with a team.
