At Medigence Health, we specialize in providing Health Risk Assessments for Medicare, Medicaid, and Commercial Marketplace members under the care of primary care physicians, medical groups, and health plans. Our focus is to conduct comprehensive assessments that align with annual wellness visit guidelines, delivered in a variety of settings including homes, clinics, via telemedicine, or in long-term care facilities. Responsibilities
Conduct thorough evaluations of the medical and social needs of beneficiary members utilizing Medigence Health's tablet-based assessment tool.
Identify and refer members for appropriate case management, whether routine, urgent, or emergent.
Provide recommendations for follow-up care with the members' primary care physicians.
Travel within designated local counties, regions, or states where you are licensed.
Be willing to reside, work, or travel to several counties and surrounding areas in Wisconsin, including Dane, Kenosha, Milwaukee, Ozaukee, Racine, Sheboygan, Walworth, Washington, Waukesha, and Winnebago.
No requirement to prescribe or modify members' current treatment plans.
Required Skills And Experience
Possess a minimum of 2 years of hands-on experience as a Nurse Practitioner or Physician.
Demonstrate knowledge of the HCC model, HEDIS, and STAR quality measures.
Hold a current, unrestricted state license or have the ability to obtain one.
Register with Medicare and Wisconsin Medicaid.
Be board certified, preferably in internal medicine, geriatrics, or family medicine.
Communicate effectively with elderly and chronically ill members.
Feel comfortable working in home visit settings and utilizing telehealth technologies.
Proficient in using computers, tablets, and mobile technology.
