Job Summary: As a Comprehensive Care Advanced Care Practitioner (ACP), you will play an integral role in our clinical team. Reporting to the Market Medical Director, you will be responsible for seeing our highest priority members, either in-center or through the Doxy.me telehealth platform. Your main objective will be to close all remaining care gaps for the calendar year in a single visit, focusing on patients with a higher level of chronic illnesses.
Pay: We offer a competitive salary range of $125,000 to $170,000, depending on experience. We are also open to discussing compensation based on your specific qualifications.
Shift: This is a full-time position with Monday to Friday, 8:00 am to 5:00 pm working hours.
Direct Hire: This is a direct hire opportunity, providing stability and growth potential within our organization.
Preferred Qualifications:
- Primary Care, Internal/Family Medicine experience.
- Experience working with the elderly population will be highly valued in this role.
- Previous experience in managed care would be a great asset.
- Clinical documentation experience.
COVID-19 Vaccine Required.
Qualifications: To be considered for this position, you must meet the following requirements:
- Hold an active, non-probationary AL medical license, or have the ability to obtain a full AL medical licensure.
- Possess a minimum of 2 years of experience working in primary care, preferably in internal medicine, geriatrics, or family medicine.
- Current BLS/ACLS certification.
- Family Nurse Practitioners, Adult Gerontology Nurse Practitioners, and Acute Care Nurse Practitioners are all welcome to apply!
